New and Evolving Immunotherapy in Inflammatory Bowel Disease

چکیده
Background: Crohn’s disease and ulcerative colitis are chronic inflammatory disorders associated with a dysregulated adaptive and innate immune response to gut commensals in genetically susceptible individuals. The pathogenesis of inflammatory bowel disease is complex, and the disease is characterized by significant phenotypic and genotypic heterogeneity. Summary: The introduction of anti-TNF biologics has resulted in improved clinical outcomes in patients with severe and moderately severe disease, but the current treatment paradigm continues to depend on systemic immunosuppression (steroids and immunomodulators) and surgical intervention in a significant number of patients, underscoring a significant unmet need. More recently, a number of genetic and immunologic abnormalities have been unraveled including aberrant intestinal mucosal defense function, abnormal intestinal permeability, dysregulated bacterial antigen processing by macrophages and presentation to T cells, cellular immune regulation and signaling, cytokine production, and leukocyte trafficking. Key Messages: Understanding these molecular mechanisms and effector pathways presents an opportunity for the development of new and improved targeted therapies. © 2016 S. Effects of Probiotics and Prebiotics in Inflammatory Bowel Disease
Inflammatory bowel diseases (IBD) including most important forms; Crohn’s disease (CD) and ulcerative colitis (UC); are chronic diseases affecting the gastrointestinal tract with unclear etiology and pathology. The role of gut microbiota has recently been considered as major a factor, since altered microbiome or "dysbiosis" is a key player in the protracted course of inflammation in IBD. Howeve...
متن کاملApplication of extracellular vesicles in the treatment of inflammatory bowel disease
Introduction: Inflammatory bowel disease(IBD) is caused by genetic, environmental, microbial and immune factors. IBD has two primary forms: Ulcerative colitis and Crohn´s disease. The incidence of IBD has significantly increased over the last few decades. Given that patients have poor response to drug treatments or are resistant to drug therapies, new therapies are needed for gastrointestinal i...
متن کاملPotential Vaccines for Treating Crohn\'s Disease
Crohn's disease (CD) is an inflammatory disease of the gastrointestinal tract (GIT) tract and can affect several parts of the digestive system. There is a relationship between impaired mucosal barrier in the GIT of inflammatory bowel disease patients and the role of bacteria such as Mycobacterium avium paratuberculosis in CD. Apart from different therapeutic approaches for treating CD, developm...

Abstract Background: Inflammatory bowel disease (IBD) is a group of inflammatory conditions of the colon and small intestine that may have critical consequences on patient’s quality of life (QOL). Many disease-specific QOL tools have been developed recently. The McMaster Inflammatory Bowel Disease Questionnaire (IBDQ) is one of them. The aim of this study was to translate into Persian and eval...
